深層層状角膜移植

はじめに

　全層角膜移植一辺倒の時代から角膜パーツ移植全盛の時代に移り変わって久しい。特に角膜内皮移植の最近の進歩は目覚ましく，手術方法の改良や新しい手術器具の開発が相次いでいる。そんななかにあって，深層層状角膜移植（deep anterior lamellar keratoplasty：以下，DALK）はやや地味な存在かもしれない。しかし，術中はopen skyとならないため安全性が高く，術後内皮型拒絶反応の心配がなく，視力の回復もよい，患者にとって恩恵の多い素晴らしい手術である。数ある角膜パーツ移植のなかでは難易度が比較的高く，習得が難しい手術であるのは否定できないが，いくつかのコツを押さえると手術の成功度は確実に増す。本稿ではDALKの基本的な部分から総おさらいをしつつ，最新情報をまじえて解説していきたい。
