インドシアニングリーン蛍光眼底造影からみた病態

はじめに

　1980年代後半から本格的に眼科で臨床応用され始めたインドシアニングリーン蛍光眼底造影では，励起光とインドシアニングリーンの発する蛍光が近赤外領域にあるために眼内組織の侵達性に優れ，フルオレセイン蛍光眼底造影では観察困難な脈絡膜循環動態や脈絡膜血管系の観察が可能である^1,2）。インドシアニングリーン蛍光眼底造影は加齢黄斑変性をはじめとしたさまざまな眼底疾患の臨床研究に用いられ，ポリープ状脈絡膜血管症や網膜内血管腫状増殖など新しい疾患概念の確立や，脈絡膜循環障害の解析に大きく貢献してきた^3～7）。

　中心性漿液性脈絡網膜症に関してもインドシアニングリーン蛍光眼底造影を用いた研究から病態の理解が飛躍的に進歩し，主病巣は脈絡膜にあり，網膜色素上皮（retinal pigment epithelium：RPE）障害は二次的変化であることが証明されてきた^8～12）。本稿ではインドシアニングリーン蛍光眼底造影の知見から中心性漿液性脈絡網膜症がどのように解釈されるようになったかを解説する。
