はじめに

 中心性漿液性脈絡網膜症(central serous chorioretinopathy:CSC)は,視力予後良好な疾患であるといわれているが,遷延例,再発例では,視力がかなり低下することも少なくない。また,漿液性網膜剝離消失後も,長期にわたり見え方の異常を訴える患者が多い。CSCによるこのような視覚異常には,漿液性網膜剝離に伴う視細胞,網膜色素上皮細胞の病態が深くかかわっている。この病態は,CSCだけではなく網膜剝離をきたす疾患に共通すると考えられ,網膜剝離に伴う網膜の変性を考えるうえでも興味深い。

 自発蛍光研究の初期から,CSCでは特異な自発蛍光がみられることが知られていた1)。自発蛍光物質の生化学的な理解が進み,眼底自発蛍光が剝離網膜下の病態を反映していることが明らかとなった。本稿では,眼底自発蛍光で明らかになったCSCで起きている網膜下の病態を中心に述べる。
