眼筋型重症筋無力症は眼科医こそが診療に当たるべきである！

はじめに

　重症筋無力症（myasthenia gravis：MG）というと，自己免疫性の全身疾患であり，眼科医が診療するなんてとんでもない，神経内科で診療するべきであると思われる先生方が多いのではないだろうか。しかし，MGは眼瞼下垂や複視・斜視など眼症状で初発するものが大部分であり，神経内科医であればしばしば初発症状の軽微な眼症状を見逃してしまう。血液検査に関しても，全身型では約80％で有用性の高い抗アセチルコリンレセプター（AChR）抗体の陽性率も，眼筋型では50％前後にすぎず^1），抗MuSK抗体^2）の陽性率も低く，眼筋型ではseronegative MGが半数近くを占める。そのため，神経内科を受診したとしてもMGの診断に至らず，適切な治療を受けられない患者も少なくない。しかも，日常臨床ではMG患者の多くは眼科を初診する^3）。したがって，初診における眼科医の責任は重大であり，決してMGを見逃してはならない。

　本稿では，眼筋型MGは眼科医が診療を担当するか，少なくとも治療方針決定に積極的に関与すべきである理由と，見逃さない診断テクニック，さらに新しい治療法について解説する。そのなかで自験の統計として引用しているのは，1994～2010年の過去17年間に兵庫医科大学病院眼科を受診し，眼科主体で治療を開始した眼筋型MG患者146例中経過を追うことのできた110例の解析結果である。
