網膜と脈絡膜の循環障害をきたしたTolosa-Hunt症候群の1例

要約　背景：Tolosa-Hunt症候群は海綿静脈洞の肉芽腫性炎症によることが多く，この部を通過する動眼神経，滑車神経，外転神経と三叉神経第1枝の障害を伴いやすい。目的：眼動脈の閉塞が生じたTolosa-Hunt症候群の1例の報告。症例：64歳男性が左側の眼窩深部痛と前頭部痛を伴う嘔吐で緊急受診した。3日前に同様の発作があり，一過性の複視があった。所見：全身状態が不良なため，視力測定はできなかった。左眼に外転と上転制限があった。その直後に急激な左眼視力低下を自覚した。矯正視力は右0.9，左0であり，左眼の網膜血管が狭窄していた。蛍光眼底造影で網膜と脈絡膜への色素流入の遅延がみられ，脈絡膜蛍光に区画状の欠損があった。左内頸動脈の海綿静脈洞部が不整で，磁気共鳴画像血管造影（MRA）で左眼動脈は造影されなかった。臨床所見と合わせ，Tolosa-Hunt症候群と診断した。ステロイドパルス療法を開始し，その翌日に左眼視力が0.3に改善した。初診から7か月後の現在まで再発はない。結論：Tolosa-Hunt症候群で一過性の網膜動脈閉塞，脈絡膜動脈閉塞が生じうることを本症例は示している。
