インフリキシマブ中止後，ぶどう膜炎の再燃と神経Behçetの顕在化を認め，再導入した1例

要約　目的：Behçet病性ぶどう膜炎がインフリキシマブ投与で寛解し，その中断後に再燃し，神経Behçetが顕在化したために治療を再開した症例の報告。症例：65歳女性が左眼ぶどう膜炎で紹介され受診した。7年前にBehçet病と診断され，コルヒチンの投与を受けていた。所見：矯正視力は右0.9，左0.07で，両眼に虹彩後癒着と白内障，左眼に眼圧上昇があった。経過：左眼への濾過手術で眼圧は正常化した。1年後に前房蓄膿が左眼に生じた。網膜血管炎が生じ，初診から4年後にインフリキシマブ投与を開始した。ぶどう膜炎の再発はなくなり，14か月後に投与を中断した。その後ぶどう膜炎が再燃し，神経Behçetの所見が生じた。インフリキシマブの再投与で眼と神経所見は寛解した。結論：インフリキシマブの全身投与がBehçet病のぶどう膜炎と神経所見に奏効した。
