ドルーゼン様色素上皮剝離からの長期経過で網膜色素上皮裂孔を生じた1例

要約　目的：ドルーゼン様色素上皮剝離が5年後に色素上皮裂孔になった症例の報告。症例：61歳男性が左眼変視症で受診した。矯正視力は右1.2，左0.9であった。右眼には集簇性軟性ドルーゼン，左眼には脈絡膜新生血管を伴う漿液性網膜色素上皮剝離があった。経過：右眼のドルーゼンは融合拡大し，5年後にドルーゼン様色素上皮剝離になり，さらに5年後に色素上皮裂孔を生じ，視力が0.2に低下した。左眼には初診の1年8か月後に網膜下出血が生じ，典型的な滲出型加齢黄斑変性の経過をとり，視力が0.03に低下した。結論：ドルーゼン様色素上皮剝離が長期間持続すると，網膜色素上皮裂孔が生じうることを，本症例の右眼は示している。
