フルオロメトロン点眼による眼圧変動

要約　目的：フルオロメトロンを継続点眼したときの眼圧変化の報告。症例と方法：過去16か月間に，鼻涙管閉塞例に対して鼻涙管チューブを挿入した128例128眼を対象とした。男性41例，女性87例で，年齢は14〜94歳，平均69歳である。手術眼に0.1％フルオロメトロンの点眼を1日4回行い，眼圧を1か月ごとに3か月後まで測定した。眼圧測定には空気眼圧計を用い，眼圧上昇が疑われる例には圧平眼圧計で確認した。眼圧が21mmHg以上で術前値よりも30％以上高い状態を，眼圧上昇と定義した。128例のうち4例が原発開放隅角緑内障として点眼加療中であり，眼圧はコントロールされていた。結果：手術前の眼圧は14.0±2.6mmHg（平均±標準偏差）であった。点眼開始後に，緑内障2眼を含む11眼（8.6％）で眼圧が上昇した。眼圧上昇は，4例では点眼開始から1か月後，7例では2か月以後に起こった。11眼中10眼では，フルオロメトロン点眼中止により眼圧が下降した。結論：0.1％フルオロメトロン点眼で，128眼中11眼（8.6％）で眼圧が上昇した。眼圧上昇を予見することは不可能であった。フルオロメトロン点眼で眼圧が上昇する可能性が周知されるべきである。
