硝子体手術の最新動向

網膜静脈閉塞症(retinal vein occlusion:RVO)は,網膜静脈分枝閉塞症(branch retinal vein occlusion:BRVO)と網膜中心静脈閉塞症(central retinal vein occlusion:CRVO)に分けられるが,それぞれの黄斑浮腫の病態に応じて,単純硝子体手術およびさまざまな追加手技の併用が試みられてきた。これらの手術は,特にわが国で積極的に行われてきた歴史があり,多くの報告がされているが,欧米ではあまり一般的ではなく,明確なエビデンスがいまだ得られておらず,議論の余地を残している。本稿では,RVOに対する硝子体手術の適応や効果に関するこれまでの報告を紹介し,抗血管内皮増殖因子(vascular endothelial growth factor:VEGF)薬など薬物療法全盛の現在における硝子体手術の意義について考えてみたい。
