はじめに

　網膜中心静脈閉塞症（central retinal vein occlusion：CRVO）は40歳以上の0.3％，65〜74歳で0.44％と高齢者によくみられる疾患であり，視機能を著しく障害する可能性が高い^1,2）。CRVOは篩状板内で中心静脈内に血栓ができることにより発症すると考えられている。これは剖検眼の報告で29眼中29眼すべてに篩状板内の血栓がみられたことが根拠となっているが，生体眼で血栓を捉えた報告はない^3）。静脈閉塞に伴い毛細血管の閉塞を起こし，蛍光眼底造影上10乳頭面積以上の無灌流領域があるものを虚血型と呼び，それ以下であるものを非虚血型としている。虚血型は高度の視力障害を生じることが多く，新生血管緑内障に至り失明することもある。700眼以上のCRVOを調査したCentral Vein Occlusion Study（CVOS）では，初めに非虚血型と判定されたものが75％，虚血型が25％であったが，非虚血型の34％が3年以内に虚血型へ移行している^4）。したがって虚血型はもちろん，非虚血型であっても篩状板後方に生じた血栓を取り除き，血流を改善させることが必要である。
