手術に至った視神経髄膜腫の小児例

要約　目的：手術的に治療した視神経鞘髄膜腫の小児の報告。症例：8歳女児に右眼視力低下が発見された。3歳のときに遠視性弱視が発見され，眼鏡装用で左右眼とも1.0の視力が得られていた。全身的に異常はなかった。所見：右眼＋7.0D，左眼＋5.75Dの遠視があり，矯正視力は右0.5，左1.2であった。対光反射は減弱し，相対的瞳孔求心路障害（RAPD）陽性で，右眼の乳頭が軽度に蒼白で中心暗点があった。磁気共鳴画像（MRI）で右視神経の腫大があった。プレドニゾロンのパルス療法には反応せず，その4か月後に中心暗点が顕著化し，発症から14か月後に右眼視力が0.04になった。MRIで鞍結節から右視神経管に伸展する腫瘍が発見され，手術で摘出した。病理所見は視神経鞘髄膜腫であった。1年後の現在まで再発はない。結論：視神経髄膜腫は，発症初期では視機能障害があっても画像診断が困難である。治療に抵抗する視神経症では，本症の可能性がある。
