はじめに
1997年，Eckardtら1）は，硝子体手術に内境界膜剝離を併用することにより，黄斑円孔の術後閉鎖率が向上することを報告した。当初，内境界膜剝離は染色剤などの補助剤なしで施行されていた。2000年，Kadonosonoら2）が，インドシアニングリーン（indocyanine green：ICG）を網膜面に塗布することにより内境界膜が染色され，内境界膜剝離を比較的容易に安全に施行することが可能であることを報告した。それ以来，ほかにも，トリパンブルー（trypan blue：TB）や，ブリリアントブルーG（brilliant blue G：BBG）などを用いて内境界膜を染色したり，トリアムシノロンを用いることにより，内境界膜剝離はより確実で安全な手技となった。
その後，黄斑上膜，黄斑浮腫，増殖性硝子体網膜症など黄斑円孔以外の病態，疾患に対しても，内境界膜剝離の有用性が報告された。近年では，巨大な黄斑円孔や強度近視に起因した難治性黄斑円孔に対するinverted flap法3），近視性牽引黄斑症に対するfovea sparing法4）など，変法も登場してきた。このように，その適応はますます拡大し，変法も登場し，硝子体手術において，本手技を併用する機会が増加していくように思われる。
一方で，内境界膜剝離は，内境界膜というMü-ller細胞の基底膜を剝離することであるため，網膜へ影響を与える可能性があり，また，染色薬剤のみでも網膜へ影響を与える可能性がある，ということは，依然から懸念されていた。実際に，dissociated optic nerve fiber layer（DONFL）（Tadayoniら2001年5））や，retinal dimpling（Spaide 2012年6））などの形態学的変化や，網膜電図上の異常などの機能的変化が，内境界膜剝離後に生じるとする報告も散見される。
本稿では，内境界膜剝離の適応，当方法に用いる補助剤，内境界膜剝離後に生じる可能性のある，網膜の形状的，機能的変化について述べる。
