遺伝性網膜疾患に対する薬物治療

遺伝性網膜疾患には，網膜色素変性（retinitis pigmentosa：RP）をはじめとして，その特殊型であるLeber先天盲やRPに感音難聴を合併するUsher症候群などの類縁疾患，卵黄状黄斑ジストロフィやStargardt病などの黄斑部機能が進行性に障害される疾患群である黄斑ジストロフィなど多彩な疾患が含まれるが，本稿ではRPを中心に述べる。
RPは，網膜の視細胞の遺伝子の突然変異により生じる緩徐進行性遺伝性疾患であり，視細胞および網膜色素上皮細胞の広範な変性がみられる原発性，進行性かつ遺伝性の疾患群である。本邦では4,000〜8,000人に1人の割合で発症し，緑内障，糖尿病網膜症に次いで成人の視覚障害原因疾患の第3位に位置する。これまで類縁疾患を含めると，80種類以上の原因遺伝子が見つかっている1）。典型例では，10〜20歳代に桿体細胞の変性による夜盲で発症し，徐々に周辺視野が障害され，視野狭窄が進行し，最終的には錐体細胞の変性により中心視力の低下から完全な失明に至る。現時点では有効な治療法は確立されていないが，近年，国内外でRPに関する病態解析研究や治療開発・研究が盛んになされてきており，病気の各段階に応じて，薬物治療，遺伝子治療，人工網膜，再生医療などのさまざまな治療戦略が考えられている。
本稿では，これまで医学的にその有効性が報告された薬物治療や，今後臨床応用が期待され，現在各国で行われている臨床試験に関して，最新の知見を交えながら，RPをはじめとした遺伝性網膜疾患の薬物治療に関して解説する。また，RPに合併する黄斑浮腫に対する薬物治療に関しても，エビデンスを交じえ解説する。
