PG製剤，ドルゾラミド塩酸塩とチモロールマレイン酸塩配合点眼液からトラボプロスト，チモロールマレイン酸塩とブリンゾラミド配合点眼液への切り替え効果

要約　目的：プロスタグランジン製剤（PG）＋ドルゾラミド/チモロール合剤（DTFC）からトラボプロスト/チモロール合剤（TTFC）＋ブリンゾラミド（BRZ）へ切り替え，またはスイッチバックした症例の眼圧変化と角膜上皮障害の報告。対象と方法：PG＋DTFC使用中の開放隅角緑内障13例24眼において，TTFC＋BRZに切り替えた場合およびスイッチバックした場合の眼圧変化，および点状表層角膜症（SPK）を検討した。結果：TTFC＋BRZに切り替え後に，眼圧は16.8±2.1mmHgから18.5±2.5mmHgと有意に上昇した。スイッチバックした症例は，16.4±1.9mmHgから切り替え後に18.5±2.5mmHgと有意に上昇し，スイッチバック後に17.1±1.9mmHgと有意に下降した。SPKのADスコアに有意な差はなかった。結論：TTFC＋BRZではβ遮断薬の持続効果が十分でない症例の存在が懸念されるため，十分な眼圧下降が得られていない症例ではPG＋DTFCへの切り替えを試みる価値がある。
