黄斑浮腫を伴った高γグロブリン血症の2例

要約　目的：黄斑浮腫を伴う高γグロブリン血症の2例を報告する。症例：症例1はIgA-κ型の多発性骨髄腫の73歳男性。両眼の黄斑浮腫により視力が低下した。右眼にトリアムシノロンテノン囊下注射（STTA）を施行したが改善せず，左眼には硝子体切除＋STTAを施行した。内科的治療で血清IgA値が低下すると黄斑浮腫は軽減し，特に左眼視力が改善した。症例2は原発性マクログロブリン血症の58歳男性。右眼の黄斑浮腫により視力が低下した。血漿交換施行した後一時的に視力が改善したが，その後網膜萎縮をきたした。結論：高γグロブリン血症に合併する黄斑浮腫の臨床経過とOCT所見を報告した。本症では，原疾患の治療に加えて硝子体手術が有効である可能性がある。
