偽黄斑円孔に対するHemi-Inverted Internal Limiting Membrane Flap Technique（Hemi-Inverted法）による硝子体手術

要約　目的：偽黄斑円孔において，円孔底網膜厚が減少し視力低下をきたす場合がある。これらに対し，黄斑上膜と内境界膜（ILM）剝離併用硝子体手術を施行した際，術後合併症に黄斑円孔がある。この場合，すでにILM剝離がされており，治療に苦慮する場合がある。今回，偽黄斑円孔に対し，Hemi-Inverted法を併用した硝子体手術の有用性につき検討した。対象および方法：対象は2011年10月〜2014年3月に当院で偽黄斑円孔に対し，Hemi-Inverted法併用硝子体手術を施行し，6か月以上経過観察可能であった11例11眼（平均年齢71.1±6.7歳，男性3例，女性8例）。手術適応基準は黄斑部偽円孔底網膜厚が100μm以下の症例とした。結果：術前平均対数視力は0.44±0.32から術後0.19±0.19（Wilcoxon signed-rank test, p＝0.0033）へ有意に改善した。術前偽円孔底網膜厚平均77.6±20.8μmから術後191.5±79.3μm（paired t-test, p＝0.00065）へ有意に増加が認められた。結論：Hemi-Inverted法は偽円孔修復に有効であり，合併症としての黄斑円孔の発生もなく，偽黄斑円孔の治療法に有効である可能性が示された。ただし，今後，症例数の増多とさらなる長期間の経過観察が必要である。
