眼内レンズ二次挿入術における眼内レンズの選択

はじめに

　近年のわが国における高齢化社会の到来により白内障手術件数も増加傾向が認められ，手術件数の増加とともに，Zinn小帯脆弱例など難症例に遭遇する機会も増えている。白内障手術の術中合併症である後囊破損に対しては，連続円形切囊（continuous curvilinear capsulorhexis：CCC）が保たれている場合は眼内レンズ（intraocular lens：IOL）の囊外固定が可能であるが，Zinn小帯が広範囲に断裂している症例に対しては，IOL縫着術や最近登場して注目を集めているIOL強膜内固定術に代表されるIOL二次挿入術が用いられる。さらに，種々の理由により，白内障術後にIOLの偏位や落下などIOL位置異常を認める症例に遭遇する機会も増えており，これらの症例に対しても同様に縫着術や強膜内固定術が行われる。縫着術は，縫着位置により毛様溝縫着術と毛様体扁平部縫着術に分けられるが，1986年の最初の報告^1）からすでに30年が経過し，その長期予後とともに適したIOLの条件が明らかとなっている。そして，わが国においても，種々の縫着用IOLが発売されて手術に用いられている。一方，強膜内固定術は2007年に初めて報告され^2），その長期予後とともに適したIOLについても不明な点が少なくない。しかし，強膜内固定術も，最近における症例の蓄積とその後の報告により次第に不明点が明らかとなってきている。本稿においては，最近の知見をもとに，縫着術や強膜内固定術などIOL二次挿入術に適したIOLについて述べる。
