動眼神経麻痺で発症した内頸動脈後交通動脈分岐部動脈瘤—発症から診断までの経緯

要約　目的：動眼神経麻痺を初発症状とする内頸動脈後交通動脈分岐部動脈瘤（以下，本疾患）に関して，発症から診断までの経緯を後方視的に調査した結果の報告。

対象と方法：対象は2003〜2012年に当科で経験した本疾患治療6例である。これら6例の発症から初診医受診までの期間，初期診断，2次施設への紹介理由などを分析した。

結果：患者側で脳疾患の自覚をもち医療機関を受診した者は1例もなかった。一方医療者側においても，1例を除き，動脈瘤の鑑別診断が上がらず，脳梗塞精査を理由に2次施設へ紹介されていた。

結論：本疾患の認知度は，医療者においても患者においても高くないので，啓発活動が重視される。
