成人男性に発症した眼窩内原発のEwing肉腫の1例

要約　目的：成人男性に発症したEwing肉腫の報告。

症例：右眼に1か月前から発症した充血と眼瞼腫脹で，43歳男性が受診した。3週間前から右眼の眼球突出と複視を自覚していた。

所見と経過：矯正視力は右0.4，左1.0で，眼圧は右27mmHg，左19mmHgであった。右眼に眼球突出と眼瞼下垂があった。MRIで右眼窩の内上縁に35mm大の腫瘤があり，頭蓋内に延長していた。全身の画像検索で他に腫瘍はなく，眼窩に原発した腫瘍であると診断した。生検と分子学的解析でEwing肉腫と診断した。腫瘍は急速に増大し，初診から5週間後に眼球が脱出した。放射線療法で眼球は復位し，化学療法で18か月後の現在まで，腫瘍の悪化はない。

結論：眼窩に原発するEwing肉腫が成人に発症することは稀である。本症例では，いったん悪化した後，放射線療法と化学療法で軽快した。
