Acremonium属真菌による難治性遅発性眼内炎の1例

要約　目的：Acremonium属真菌による硝子体手術後に発症した眼内炎の報告。

症例：75歳女性が眼内炎として紹介受診した。3週前に右眼の網膜前膜に対し，硝子体手術と水晶体再建術を他医で受けていた。

所見と経過：矯正視力は右0.7，左1.2であり，右眼の前房に細胞浸潤と強角膜切開創からフィブリン析出があった。術後遅発性眼内炎と診断した。前房を洗浄し，抗菌薬を眼内と全身に投与して，前房所見は軽快した。1か月後に眼内炎が再発し，前房洗浄または硝子体手術を11回行った。前房水または硝子体の細菌，ウイルス，真菌についての培養は，一貫して陰性であった。初診から27週後に瞳孔縁に白色塊が生じ，培養でAcremonium属の真菌が検出された。ボリコナゾールの内服と点眼で眼内炎は治癒した。毛様体扁平部の白色塊の増殖で網膜剝離が生じ，指数弁となった。

結論：Acremonium属の真菌による眼内炎は，経過が緩徐で難治性であったが，ボリコナゾールの点眼と全身投与が奏効した。
