眼内悪性腫瘍の諸問題—ぶどう膜メラノーマを中心に

　ぶどう膜メラノーマは成人に発症する代表的な眼内悪性腫瘍の1つである。自験例の検討では，本症の5年生存率は76％，10年生存率は57％であり，転移をきたした後の生命予後が厳しい現実は今も変わりはない。

　メラニン産生細胞と親和性のある¹²³I-IMPを用いた核医学検査は感度・特異度ともに優れた診断法であり，眼内液中の液性因子の解析は良性色素性眼内腫瘍との鑑別に有用な可能性がある。治療には眼球摘出術のほか，小線源照射や重粒子線照射，局所切除術などの眼球温存療法があるが，それぞれの長所・短所を理解して実施する必要がある。本症の予後は一部の染色体（遺伝子）異常によって規定されることが知られているが，邦人を対象とした筆者らの検討でも欧米諸国の報告と同様の傾向が確認されている。

　肝などへの転移により死に至る可能性のあるぶどう膜メラノーマの診断と治療については常に慎重な対応が求められる。
