網膜中心静脈閉塞に伴う黄斑浮腫に対するアフリベルセプト硝子体注射の治療成績

要約　目的：網膜中心静脈閉塞（CVO）に伴う黄斑浮腫に対するアフリベルセプト硝子体注射（IVA）の効果の報告。

対象と方法：CVOの黄斑浮腫にIVAを行い，6か月以上観察可能であった21眼を対象とし，治療後1，3，6か月の視力と中心網膜厚（CRT）について検討した。対象の内訳は，初回治療が13眼，ベバシズマブからの切り替えが8眼である。

結果：治療前視力の平均logMAR値は0.72で，治療1，3，6か月後には0.45，0.49，0.43と改善した。治療前のCRTは727.2μmで，1，3，6か月後には260.5，287.8，333.2μmと，いずれも有意に改善した（p＜0.01）。6か月での注射回数は平均3.1回であった。95％の症例では浮腫が消失したが，再発が95％にあり，67％は注射後2〜3か月に再発し，10％は5か月以上後に再発した。

結論：CVOにIVAは短期的には虚血のタイプによらず有効であるが，再発が多く，より長期の注意深い観察と追加治療が必要である。
