Descemet membrane endothelial keratoplasty（DMEK）における移植片折れ曲がり整復テクニック

要約　目的：Descemet membrane endothelial keratoplasty（DMEK）の周術期合併症の1つに移植片接着不良があり，移植片生存率に大きくかかわる。その合併症を回避する手術手技を考案したので，症例とともに報告する。

対象と方法：対象はレーザー虹彩切開術後水疱性角膜症の6例6眼（女性6眼，平均年齢70.8歳）。DMEK移植片を前房内で展開後，角膜後面に接着させる際に生じる移植片の折れ曲がりを正常形状へ回復する方法としてbubble-bumping maneuverが既報にある。この操作を全象限に対して追加してDMEKを行った。

結果：全例移植片接着不良に対する空気再注入処置を要することはなく，良好な移植片接着が得られた。

結論：本法はDMEK移植片接着不良を回避するうえで有用である。
