血管新生緑内障に対するバルベルト^®緑内障インプラントの手術成績

要約　目的：血管新生緑内障に対するバルベルト^®緑内障インプラントを挿入した成績の報告。

対象と方法：過去3年間にバルベルト^®緑内障インプラントを挿入した25例27眼を対象とした。男性19眼，女性8眼で，年齢は29〜82歳，平均61歳である。糖尿病網膜症が23眼，網膜中心静脈閉塞症が3眼，網膜静脈分枝閉塞症が1眼にあり，全例に新生血管が虹彩または隅角にあった。硝子体手術など手術の既往が22眼にあった。全例で6か月以上，平均17か月の経過を追った。最終眼圧が5mmHg以上，21mmHg以下の場合を成功と定義した。

結果：眼圧の平均値は，術前36.2±11.4mmHgから，1年後11.1±3.8mmHg，2年後12.3±4.5mmHgと有意に下降した。累積成功率は術1年後90.1％，2年後90.1％であった。術後1か月以内に，合併症として前房出血が7眼，硝子体出血が2眼，脈絡膜剝離が2眼に生じた。術後1月以降に，30％以上の角膜内皮細胞密度減少が6眼にあった。

結論：血管新生緑内障に対するバルベルト^®緑内障インプラント挿入は，約90％で奏効した。
