脈絡膜母斑に合併したポリープ状脈絡膜血管症の1例

要約　目的：脈絡膜母斑に併発したポリープ状脈絡膜血管症の1例の報告。

症例：61歳の女性が4か月前からの右眼の変視症で受診した。矯正視力は右0.1，左1.2で，右眼の黄斑部より鼻上側に扁平で淡い黒色の病変，網膜色素上皮剝離と漿液性網膜剝離があった。光干渉断層計などの所見から，脈絡膜母斑に併発したポリープ状脈絡膜血管症と診断した。ラニビズマブ硝子体注射が6回，アフリベルセプト硝子体注射が14回行われた。改善と悪化を繰り返し，3年後の視力は0.6で維持されている。

結論：脈絡膜母斑によって脈絡膜循環障害が生じ，ポリープ状脈絡膜血管症になったと推定される。抗VEGF薬の硝子体注射が奏効したが，複数回の治療が必要であった。
