Uveal effusion syndromeにおける網脈絡膜の光干渉断層計による観察

要約　目的：Uveal effusion syndrome（UES）2症例のスペクトラルドメイン光干渉断層計（SD-OCT）による所見の報告。

症例：第1例は67歳女性で，左眼黄斑部の網膜剝離で受診した。15年前に両眼に網膜剝離を伴う脈絡膜剝離が発症した。UESと診断され，右眼に2回，左眼に1回の強膜開窓術が行われ，寛解していた。眼軸長は左右眼とも約16mmであった。第2例は39歳男性で，24年前に両眼にUESが発症し，右眼に4回，左眼に5回の強膜開窓術が行われ，寛解していた。眼軸長は左右眼とも約23mmであった。

所見と経過：活動期にあると推定される第1例では，SD-OCTにより両眼に顕著な脈絡膜肥厚があり，脈絡膜内層から中層に点状高反射が多発していた。右眼には網膜外層が消失し，outer retinal tubulationがあり，左眼には脈絡膜反射が増強していた。陳旧期にあると推定される第2例では，右眼のellipsoid zoneが保たれ，脈絡膜肥厚と脈絡膜内層から中層に点状高反射が多発していた。左眼では網脈絡膜の菲薄化，網膜外層の消失，網膜色素上皮から脈絡膜の薄い高反射帯，脈絡膜反射の増強があった。

結論：UESの陳旧期には，網膜外層と網膜色素上皮の障害を示す所見があった。活動期と陳旧期にある脈絡膜の点状高反射は，高密度の脈絡膜血管に相当すると解釈された。
