光線力学療法とベバシズマブ硝子体注射により長期間視力が改善した脈絡膜骨腫に続発した脈絡膜血管新生の1例

要約　目的：脈絡膜骨腫では，約半数で脈絡膜新生血管などにより視力が低下するとされている。脈絡膜骨腫に続発した脈絡膜新生血管に対し，1回のベバシズマブ硝子体注射と光線力学療法で視力が改善し，長期に維持された症例の報告。

症例：17歳の女性が2週間前からの左眼視力低下と中心暗点で紹介受診した。紹介医により左眼黄斑部に漿液性網膜剝離が指摘された。

所見と経過：矯正視力は右（1.2），左（0.4）で，右眼に−5D，左眼に−6Dの近視があった。左眼後極部に境界が明瞭な黄白色の病変があった。CTで眼球後壁に石灰化があり，諸検査の結果，脈絡膜新生血管を伴う脈絡膜骨腫と診断した。ベバシズマブ硝子体注射と光線力学的療法を行い，1か月後に出血は消失し，10か月後に視力は（1.2）に改善した。2年後の現在，左眼の眼底病変は沈静化し，（0.8）の視力を維持している。

結論：脈絡膜骨腫に続発した脈絡膜新生血管に対する1回のベバシズマブ硝子体注射と光線力学療法で，視力が改善し，以後2年間眼底病変が鎮静化している。
