IgG4関連疾患と視神経症

はじめに

　IgG4関連疾患とは，血清のIgG4が上昇し，全身のさまざまな臓器や器官にリンパ形質細胞浸潤病変がみられる病態である。その疾患概念は2001年のIgG4関連自己免疫性膵炎の報告^1）により誕生し，また眼領域では2004年に初めてIgG4関連ミクリッツ病が報告された^2）。その後の症例の蓄積を経て，2015年に本邦から公表されたIgG4関連眼疾患の診断基準では，その3大病変として涙腺，三叉神経，外眼筋の腫大が示された^3）。一方，それらに比較して発症頻度は低いものの，最も重篤で留意すべき病態は視神経症である。他の視神経炎や緑内障との鑑別の観点からも，IgG4関連視神経症という病態の存在を知っておくべきである。
