緑内障薬の点眼時間の変動とアドヒアランス

要約　目的：緑内障患者において，緑内障薬の点眼時間の変動とアドヒアランスの関係を検討した。

対象と方法：対象は，少なくとも6か月にわたり，ラタノプロスト点眼液0.04％あるいはラタノプロスト・マレイン酸チモロール配合点眼液を単独に点眼している緑内障患者324名（70.6±11.6歳，男性138名，女性186名）である。面接調査によって，点眼時間とその変動および点眼のアドヒアランスを調査した。

結果：全例の平均点眼時間の変動幅は60.0±74.3分であった。アドヒアランス良好な患者の点眼時間の変動幅は52.3±62.1分で，不良な患者では78.3±94.3分であった（p＜0.0001）。アドヒアランス良好な患者は，点眼時間の変動が240分以上で6名（46.1％），120〜239分で20名（57.1％），60〜119分で83名（72.2％），30〜59分で75名（74.2％），15〜29分で10名（83.3％），15分未満で39名（81.5％）であり，変動が小さくなるにしたがい，アドヒアランス良好な患者の割合が有意に上昇した（p＜0.0001）。点眼時間は決めていないと回答した患者の点眼時間の変動幅は227.5±161.0分であり，他の患者に比較して有意に長く，アドヒアランスも有意に不良であった（p＜0.0001；p＜0.0001）。

結論：緑内障薬の点眼時間の変動が大きくなるほど，点眼のアドヒアランスが有意に不良になった。
