外転神経麻痺が初発症状であった胆管癌の脳転移の1例

要約　目的：外転神経麻痺発症をきっかけに胆管癌の脳転移が発見された1症例を報告する。

症例：73歳の女性が2か月前から突然生じた無痛の左眼の内斜視の精査のため当科を紹介され受診した。胆管癌の既往があり紹介元の当院外科で撮った直近のMRIでは異常がないと判定されたが，6週間後に造影MRIを施行したところ鞍上部に転移性腫瘍が見つかった。

初診時所見：左眼は60Δ以上の内斜視で外転制限があり，左眼の矯正視力は0.6であった。両眼とも眼底所見は正常で，対光反応正常，RAPD（−）で，眼球運動時痛はなかった。

経過：転移性脳腫瘍が見つかった後の1か月の間に左眼の眼瞼下垂，構語障害，頭痛が出現し，ガンマナイフ治療をしたが癌性髄膜炎により死亡に至った。

結論：急性の斜視においては眼球の神経麻痺症状をみたら脳腫瘍や脳動脈瘤を第一に疑って精査する必要がある。特に悪性腫瘍の既往がある場合には躊躇せずに造影MRI検査を選択すべきである。
