放射状角膜切開術20年後に鈍的外傷による重篤な眼球破裂をきたした1例

要約　目的：近視に対する放射状角膜切開術の20年後に，鈍的外傷による重篤な眼球破裂が生じた症例の報告。

症例：41歳男性が自宅で転倒し，右眼の打撲後に視力障害を自覚して受診した。20年前に近視に対する放射状角膜切開術を他医で受けていた。

所見と経過：裸眼視力は右光覚弁，左眼1.0であった。右眼の角膜に放射状の切開創が8本あり，うち2，4，8時方向の切開創が離開し，その一部から硝子体が脱出していた。前房は消失し，水晶体はなかった。左眼にも放射状角膜切開術の術創があった。切開は角膜厚の90％を超え，ほとんどデスメ膜に達していた。左眼角膜内皮細胞の密度は2,662/mm²であった。右眼の角膜創を縫合したが，硝子体出血のため，最終視力は光覚弁であった。

結論：放射状角膜切開術の既往がある眼では，長期経過後であっても，軽度な外傷を契機に角膜創が離開することがある。
