トーリック眼内レンズの軸ずれの屈折への影響

要約　目的：トーリック眼内レンズ（IOL）の軸ずれが屈折に与える影響の自覚値および理論値による解析。

対象と方法：トーリックIOLが明らかに軸ずれした9眼を対象として，軸ずれ時の自覚的屈折値を，挿入したIOL度数，軸ずれ角度，および術後のケラト値などから計算した理論値，および軸ずれがないと仮定した理論値と比較した。

結果：軸ずれ角度の平均は37.1±21.1°（最小13°，最大81°）であった。軸ずれ時の自覚的屈折値および理論値の球面度数は1.03±0.49Dおよび0.86±0.76Dで，軸ずれがない理論値の0.18±0.55Dと比較して有意に遠視になっていた（ともにp＜0.01）。自覚的屈折値および理論値の円柱度数は2.50±0.48Dおよび2.14±0.66Dと，軸ずれがない理論値0.80±0.50Dと比較し，有意に乱視が強かった（ともにp＜0.01）。また等価球面値の軸ずれ時の自覚値は，理論値および軸ずれがない理論値と比べ（p＝0.47，0.48），ともに有意差はなかった。

結論：トーリックIOLが軸ずれすると，乱視矯正効果が減弱するだけでなく，球面度数が遠視化し，乱視軸が変化する。
