側頭部への美容目的ヒアルロン酸注入により網膜動脈閉塞をきたした1例

要約　目的：眉間および鼻唇へのヒアルロン酸製剤や自己脂肪組織の誤注入により網膜動脈閉塞をきたすことが報告されている。側頭部へのヒアルロン酸製剤注入により網膜動脈閉塞をきたした1例を経験したので報告する。

症例：34歳女性。左側頭部に美容目的にてヒアルロン酸製剤を注入した直後から左眼痛と急激な視力低下を訴えた。

所見：左眼の矯正視力は0.05，眼圧は12mmHgであった。眼底は多発性の乳白色化病変を呈し，蛍光眼底造影検査にて網膜動脈内に多数の塞栓を認め，網膜動脈分枝閉塞症と診断した。保存的加療にて矯正視力は1.2まで改善したが，視野障害を残した。

結論：過去に報告のない側頭部の動脈への誤注入によって網膜動脈閉塞が生じたと考えられた。
