トラベクレクトミー術後のデレンに対する治療

今月の症例

【患者】51歳，男性

【主訴】左眼の眼痛

【現病歴】19歳時に緑内障と診断され，両眼の線維柱帯切除術を受けた。右眼はその時点で失明した。左眼は半年前より眼圧が再上昇し，手術目的で当院紹介となった。初診時，左眼の眼圧は52mmHg，隅角はwide openであり，緑内障手術が必要と判断した。左眼の11時方向に緑内障手術痕を認めたため，2時方向に線維柱帯切除術を施行した（図1）。術中・術後の経過は良好であったが，術3か月後に左眼の眼痛を訴え，再受診した。
