黄斑円孔手術におけるinverted ILM flap techniqueの評価

はじめに

　黄斑円孔は比較的中年に発生する代表的な眼底疾患であり，その治療に硝子体手術を要する。手術としては，可及的硝子体切除，人工的後部硝子体剝離（posterior vitreous detachment：PVD）作製，内境界膜（internal limiting membrane：ILM）剝離，ガスあるいは空気によるタンポナーデが広く施行されるようになり，術後の初回円孔閉鎖率が良好な時代になった。しかしながら，最小円孔径が400μm以上の大型の黄斑円孔や陳旧性黄斑円孔ではこれらの手術治療を行っても，円孔閉鎖が得られず難治性となることが経験される。

　図1に筆者が経験した黄斑円孔非閉鎖の70歳代男性の1例を示す。初診時，stage 3の黄斑円孔がみられ（図1a，b），硝子体手術を検討したものの肝臓癌の治験中で手術を延期した。初診1年後に治療可能となったが，依然stage 3で明らかな円孔径の拡大はなかった（図1c，d）。型通りILM剝離併用の硝子体手術，ガスタンポナーデを行ったが，初回円孔閉鎖はできなかった（図1e，f）。このような症例は，陳旧性黄斑円孔に相当すると考えられ，網膜の伸展は不良化し，難治性となる。

　2010年に報告されたinverted ILM flap technique（法）は画期的な手法であり，このような大型の黄斑円孔に対して初回円孔閉鎖率のさらなる向上に貢献する術式である^1）。この術式は，かつての硝子体手術の際にアーケード内のILMを剝離除去する手法ではなく，円孔周囲のILMを全周性に残して翻転し，円孔上に被覆する術式である。大型の黄斑円孔に加えて，ぶどう膜炎^2）や網膜静脈閉塞症に伴う二次性黄斑円孔，陳旧性黄斑円孔，さらには黄斑円孔網膜剝離に対しても，良好な初回円孔閉鎖率を得ることが可能になってきている。

　本稿では，誌面の関係上inverted ILM flap 法の評価として，従来のILM剝離との比較，術中および術後評価および本法の適応，課題について述べたい。
