黄斑円孔手術による視野障害

はじめに

 硝子体手術の急速な進歩によって手術がより低侵襲となり,より良好な術後視機能が得られるようになってきた。それに伴い,術前視機能が比較的良好な症例に対しても手術適応が拡大される傾向にある。しかし,硝子体手術にはさまざまな合併症があり,それによって視機能障害が生じる可能性がある。術後の視野障害に関して,黄斑円孔術後に視野障害が生じることは,1995年にMelbergらが初めて報告した1)。それ以後,複数のグループがさまざまな原因によると思われる術後視野障害を報告している。視野障害の原因として報告されてきた主なものとして,液空気置換,生体染色,そして内境界膜(internal limiting membrane:ILM)剝離が挙げられる。
