以前の常識

　●マイボーム腺機能不全（MGD）の診断は眼瞼縁の所見，マイボーム腺分泌脂（meibum）の色や粘度など，検者の主観や熟練に頼るものが多く，検者によるばらつきが大きかった。

　●MGDの治療はステロイド眼軟膏，抗菌薬眼軟膏，人工涙液点眼など対症療法が中心に処方されてきたが，長期的に効果を実感できるものがほとんどなく，患者の満足を得られないことが多かった。

現在の常識

　●MGDの診断はマイボグラフィ（マイボーム腺の形態観察），インターフェロメトリー（涙液油層観察）の出現により客観的に再現性高く診断することができ，検者によるばらつきが減少した。治療効果のモニタリングも可能となった。

　●MGDの治療は温罨法やリッドハイジーンの有効性を示すエビデンスが次々と示され，“脂が足りないタイプ”のドライアイをターゲットにした治療法が次々とリリースされ，根治を目指すものとなっている。
