以前の常識

　●IgG4関連疾患の眼症状は，古来よりMikulicz病と称される対称性涙腺腫大が主であり，重篤な視機能障害を呈する病態は知られていなかった。

　●IgG4関連涙腺炎からリンパ腫が発症した症例報告が散見されたが，眼窩リンパ増殖性疾患に占めるIgG4関連疾患の割合など，その頻度は不明であった。

現在の常識

　●IgG4関連疾患の眼症状には涙腺腫大のほかにも三叉神経腫大，外眼筋腫大がしばしばみられ，これらはIgG4関連眼疾患の3大病変である。また，視神経症による重篤な視機能障害をきたす症例もある。

　●日本での多施設調査では，IgG4染色陽性MALTリンパ腫の症例数はIgG4関連眼疾患のおよそ1/5であり，治療に際して両者の鑑別は重要である。
