Paracentral acute middle maculopathyを呈した5例

要約　背景：Paracentral acute middle maculopathy（PAMM）とは，急性の視力・視野障害が生じ，光干渉断層計（OCT）で網膜中間層に高輝度病変が生じる病態であり，網膜深層の毛細血管網の虚血が原因と考えられている。

目的：過去11か月間に経験したPAMM 5症例の報告。

症例：3症例は男性，2症例は女性で，年齢はそれぞれ39，52，69，78，81歳である。いずれも片眼性であり，3例は前医で網膜動脈分枝閉塞症，1例はVogt-小柳-原田病（原田病）と診断されていた。1例は当院に両眼網膜静脈閉塞症にて通院中であった。主訴は視野の中心または傍中心部の見えにくさであった。

所見と経過：患眼の診断時視力は，0.6〜1.0の範囲にあった。3眼では中心窩近くに浮腫状の色調変化があり，5眼すべてにOCTで中心窩近くに網膜中間層の帯状の高輝度病変があった。フルオレセイン蛍光眼底造影では，これに相当する異常所見はなかった。以上より5例すべてをPAMMと診断した。3〜6か月後に中心窩近くの色調変化は軽快し，OCTで同部位の網膜中間層が菲薄化した。原田病が当初疑われた例では，抗リン脂質抗体症候群の既往があることが判明した。

結論：今回の5症例でのPAMMは急性の視力または視野障害で発症し，中心窩近くの色調変化を呈した。蛍光眼底造影所見は正常であり，OCTがその診断に有用であった。
