急激に増大した角膜ケロイドに対して病理学的検討を行った1例

要約　目的：角膜ケロイドは，緩徐に増大する角膜中央部に生じる良性の結節性病変であり，過去に100症例ほどの報告がある。今回，急激に増大する角膜ケロイドの1症例を経験し，病理学的な検討を行った。

症例：10歳の男性。幼少時より右眼に角膜混濁を認め，視力が0.01と低下していることにより，原因精査目的で当院を紹介され受診した。右眼の角膜中央部に軽度に隆起した結節性病変を認めた。左眼の角膜にもわずかに血管侵入と上皮下混濁を認めた。原因を精査中に患者の通院は途絶えた。初診より8年後，右眼の角膜混濁が急激に増大してきたため，再度当院を紹介され受診した。右眼の結節性病変は8年前と比較して増大し，大きく前方に突出していた。患者は審美的な改善を希望していたため，角膜表層切除術を施行した。

所見：病理学的に，肥厚した角膜上皮，線維血管組織の増殖，局所的な血管外へのフィブリン析出を認めた。免疫組織化学的に線維芽細胞はα-smooth muscle actin陽性，vimentin陽性であり，活性の高さを示していた。

結論：急激に増大する角膜結節性病変として角膜ケロイドも鑑別に入れる必要がある。角膜ケロイドが急激に増大する機序としては，線維血管組織の増殖だけではなく，血管透過性の亢進も関与すると考えられた。
