斜視術中強膜穿孔後に生じた裂孔不明網膜剝離の1例

要約　目的：斜視手術中に強膜穿孔をきたし，術後難治性裂孔不明網膜剝離を生じた1例を報告する。

症例：72歳，男性。局所麻酔下での右眼外斜視術中に強膜穿孔をきたしたが，穿孔部を冷凍凝固し手術を続行した。術後3か月に網膜剝離を認めた。硝子体手術中に冷凍凝固斑部位および全周鋸状縁まで観察したが，裂孔はなかった。意図的裂孔を作製し，高粘稠度の網膜下液を排出した。術後上耳側最周辺部に限局した網膜下液を認め，剝離は徐々に拡大した。7か月後剝離の範囲が意図的裂孔まで広がったため，再度硝子体手術を行った。強膜穿孔部位は瘢痕化し，前回同様に明らかな裂孔は検出されず，高粘稠度の網膜下液を排出した。術後6か月の経過観察では最周辺部に網膜下液が残存しているものの，範囲は拡大せず，視力は0.7を維持している。

結論：裂孔が検出されないため，過剰冷凍凝固および炎症の持続による滲出性網膜剝離が疑われた。
