ぶどう膜炎に続発した血管新生緑内障2症例の前房内VEGF濃度と治療経過

要約　目的：血管新生緑内障（NVG）は糖尿病網膜症や網膜静脈閉塞症以外にぶどう膜炎にも続発しうるが，ぶどう膜炎には種々の病態が含まれ網膜循環障害をきたさない場合もある。網膜虚血を認めないぶどう膜炎によりNVGを呈した2症例の前房内血管内皮増殖因子（VEGF）濃度を測定し，ベバシズマブ硝子体内注射（IVB）を併用し加療した経過を報告する。

症例：症例1は55歳，女性。約1年半放置後のぶどう膜炎およびNVGにて紹介された。左眼に著明な前房炎症・虹彩新生血管・虹彩後癒着を認めた。IVBおよびステロイド局所投与や降圧治療後，白内障手術併用隅角癒着解離術を施行した。術後蛍光眼底造影にて網膜循環障害はなかった。症例2は69歳，女性。初診時の右眼圧は58mmHgであり，右眼虹彩・隅角の新生血管が著明で前房・硝子体内炎症所見と広範な網膜萎縮瘢痕巣を認め，蛍光眼底造影にて明らかな網膜灌流障害はなかった。IVBおよびステロイド点眼・内服，降圧治療のうえ，線維柱帯切除術を施行した。

結果：IVB前の前房水VEGFは順に1,220，1,080pg/mlと高値で，IVB後は速やかにルベオーシスが消退し，周術期出血性合併症はなかった。

結論：網膜に虚血性病変を認めないぶどう膜炎に続発したNVGにおいても，前房内VEGF濃度高値が確認された。ステロイドの効果も考慮されるが，IVB後ルベオーシスは迅速に消退し，治療上有用性があると考えられる。
