斜視に対するボツリヌス毒素治療

POINT

●斜視に対するボツリヌス治療は2015年6月に保険適用となった。外眼筋にボツリヌス毒素を注射することで筋の収縮力を弱め，後転術に似た効果を得ることができる。

●本療法の良い適応として，急性後天性内斜視，麻痺性斜視の急性期，小角度の甲状腺眼症，手術後のシミュレーションが必要な例，大角度の斜視に対する後転術との併用がある。

●効果の持続期間は3〜4か月である。本療法の利点は，急性期の斜視に介入可能なこと，侵襲が少なく，短時間の処置で済むことである。欠点は，手術に比べ定量性に乏しく効果が不安定であること，一過性眼瞼下垂や一過性上下斜視の副作用などである。効果が一過性であることは，利点とも欠点ともなりうる。
