眼内悪性リンパ腫

POINT

●眼内悪性リンパ腫は代表的な仮面症候群であり，中枢神経病変を合併した場合には予後不良となることが多く，発症早期における確実な診断が重要である。

●硝子体検体を用いた細胞診に加えて，フローサイトメトリー，免疫グロブリン重鎖遺伝子再構成の確認，サイトカインの測定など，多角的に診断する。

●中枢神経や全身の病変を合併する場合には全身化学療法が中心となり，眼内に限局する場合はメトトレキサート硝子体注射や放射線治療が行われるが，後者に対する全身化学療法の是非については議論の分かれるところである。
