トーリック眼内レンズの固定位置と回旋

Q　図1のように，トーリック眼内レンズ（intraocular lens：IOL）を移植しましたが，回旋してしまいます。レンズの位置を何度補正しても，回旋してしまいます。トーリックIOLの固定位置や固定方法について教えてください。そもそもトーリックIOLは必要なのでしょうか？
