網膜色素変性症に中心性漿液性脈絡網膜症を併発した1症例

要約　目的：網膜色素変性症（RP）眼は進行性に網脈絡膜萎縮を起こすので，中心性漿液性脈絡網膜症（CSC）を発症しにくいとされている。今回筆者らは，RPにCSCを併発した症例を経験したので報告する。

症例：症例は37歳，男性。健康診断で右眼の視力低下を指摘され近医を受診し，RPの診断で精査加療目的に当科を初診した。初診時視力は，右（0.5），左（1.5），眼圧は左右ともに15mmHgであった。両眼とも周辺赤道部に網膜血管の狭小化，骨小体様の色素沈着を伴う網脈絡膜萎縮を認めた。網膜電図にて錐体・杆体応答ともに振幅の低下を認めた。右眼黄斑部に漿液性網膜剝離（sRD）を認め，フルオレセイン蛍光眼底造影で蛍光漏出点を認めた。以上よりRPに併発するCSCと診断した。CSCに対しては，蛍光漏出点に網膜光凝固術を行った。いったんはsRDは改善したものの，再発を繰り返し，その後も複数回の網膜光凝固術を要した。

結論：RP患者でのCSC発症は稀であるが，いったん発症すると治療に苦慮する可能性がある。
