脈絡膜新生血管を合併した散弾様網脈絡膜炎の1症例

要約　目的：散弾様網脈絡膜炎（BC）は欧米人に多くみられる，多数の黄白色滲出斑を特徴とするぶどう膜炎の1型である。約10％に脈絡膜新生血管（CNV）が発生するとされているが，本邦でCNVが合併した報告は過去に1例しかない。筆者らは，本症の経過中にCNVをきたした症例を経験したので報告する。

症例と経過：54歳，男性。主訴は両眼霧視，初診時所見は両眼視神経乳頭発赤，網膜静脈の拡張・蛇行，黄白色滲出斑が後極部に多発していた。ステロイド点眼と内服による治療を開始したが，ぶどう膜炎が再発し，両眼眼底周辺部全体に多数の黄白色滲出斑が出現した。眼所見が本症に特徴的で，全身検査で他のぶどう膜炎が否定的であったため，BCと診断した。その後寛解と増悪を繰り返したが，初診より1年3か月後に右眼，その2か月後には左眼にもCNVが発生した。抗血管内皮増殖因子（VEGF）薬硝子体内注射および，トリアムシノロンテノン囊下注射（STTA）を必要時投与で行ったところ，CNVは徐々に拡大したものの，瘢痕化した。

結論：本例のCNVはぶどう膜炎の慢性炎症により発生したと思われた。治療にはSTTAと抗VEGF薬硝子体内注射が有効であったが，CNVは拡大し視力が低下したため，より積極的な治療が必要であると思われた。
