シクロスポリン併用経口ステロイド療法が有効であった特発性外眼筋炎の1例

要約　目的：原因不明の良性かつ非感染性炎症性疾患である特発性眼窩炎症のうち，炎症の首座が外眼筋にあるものを特発性外眼筋炎という。治療はステロイドを使用することが一般的であるが，再発を繰り返す難治症例も多い。今回筆者らは，ステロイドだけでは再発を繰り返す難治性の特発性外眼筋炎に対して，ステロイドパルス療法後の経口ステロイド療法にシクロスポリンを併用することで再発を抑えることができた症例を経験したので報告する。

症例：38歳，女性。38歳時に右外直筋炎を発症した。経口ステロイド療法でいったん改善したが，39歳時に左内直筋炎（経口ステロイド療法とステロイドパルス療法で加療），40歳時に左外直筋炎を発症した。経口ステロイド療法だけではコントロールがつかず，41歳時にステロイドパルス療法を施行した。後療法として経口ステロイド療法に切り替えた直後に再発したため，追加のステロイドパルス療法を施行した後に経口ステロイド療法にシクロスポリンを併用したところ，再発することなく経過した。

考察：特発性外眼筋炎に対してシクロスポリンを使用した症例は既報に乏しい。本報告は，再発性の特発性外眼筋炎に対して，難治性ぶどう膜炎の治療と同様，ステロイドに加えてシクロスポリンを併用することが治療の一選択肢として有用である可能性を示すものである。
