α_1A受容体遮断薬内服者における術中虹彩緊張低下症候群出現と虹彩形状の関連についての検討

要約　目的：α_1A受容体遮断薬内服者の虹彩形状と，術中虹彩緊張低下症候群（IFIS）出現との関連の報告。

対象と方法：新長田眼科病院で白内障手術を施行したα_1A受容体遮断薬（タムスロシン塩酸塩，ナフトピジル，シロドシン）内服者20例35眼を対象とした。IFIS発症群と非発症群において，前眼部光干渉断層計（OCT）で測定した散瞳前後の瞳孔径・瞳孔括約筋厚・瞳孔散大筋厚・虹彩面積を比較した。

結果：IFISを発症したものは10例16眼であり，そのうち完全型は5例6眼，不全型は8例10眼であった。完全型はすべてシロドシン内服例であった。IFIS発症群/非発症群の散瞳前の瞳孔径は2.82/3.43mm（p＜0.05），括約筋厚は0.48/0.50mm，散大筋厚は0.41/0.48mm（p＜0.05），虹彩面積は2.07/2.17mm²，散瞳後の瞳孔径は5.01/6.04mm（p＜0.05），括約筋厚は0.48/0.55mm，散大筋厚は0.46/0.52mm（p＜0.05），虹彩面積は1.65/1.66mm²（すべて平均値）であり，散瞳前後の瞳孔径と散大筋厚に有意差があった。

結論：α_1A受容体遮断薬内服者のIFIS発症率は45％で，シロドシン内服例ではIFISが好発し，重症化する傾向があった。IFIS発症は瞳孔散大筋厚が薄いこと，散瞳不良であることと有意に関連していた。IFIS発症のリスク評価に前眼部OCTが有用である可能性がある。
