診断に苦慮したノカルジア強膜炎の1例

要約　目的：トリアムシノロンテノン囊下注射施行後に強膜炎を発症し，診断に苦慮したノカルジア強膜炎の1例を経験したので報告する。

症例：82歳，女性。近医にて左網膜静脈分枝閉塞症，黄斑浮腫に対してトリアムシノロンテノン囊下注射を施行した。注射の7週間目に強膜炎を発症し，抗菌薬とステロイドの点眼治療を開始したが改善せず，千葉大学医学部附属病院眼科に紹介となった。

所見：左眼の耳側結膜に充血と強膜壊死を伴う結節性病変がみられた。前房内には炎症細胞とフィブリンの析出を認めた。抗菌薬とステロイドの点眼，内服を開始後，症状は改善傾向にあったが，その後増悪し，左眼窩膿瘍も出現した。強膜の結節性病変から得られた膿よりNocardia elegansが検出されたため，ノカルジア強膜炎と診断した。スルファメトキサゾール・トリメトプリムの内服治療を開始したところ，症状は速やかに改善した。

結論：ステロイドのテノン囊下注射を含む処置後に生じた難治性強膜炎では，ノカルジア強膜炎の可能性も念頭に置いて治療にあたることが重要である。
